About this Webinar

20% of Medicare members/patients struggle with self-managing their heath due to complex diagnoses such as dementia, stroke, Parkinson’s, or ESRD.  These difficult-to-engage members incur more than 50% of total healthcare costs.  Increasingly, family caregivers, who are uniquely able to detect changes in a member’s condition before it worsens, are being enlisted to engage the healthcare system on behalf of a loved one.  Join this discussion, sponsored by Ceresti Health, to learn how a digital caregiver activation program can improve care and outcomes for these complex and underserved Medicare members.

This session is intended for payers and provider organizations that manage risk in Medicare.  The following topics will be covered:

  • Overview of caregiving landscape
  • Benefits of family caregiver inclusion into the care team
  • Digital caregiver activation program, including outcomes (cost savings, increased RAF accuracy and increased member experience)
  • Implementing programs to support family caregivers

California-based Ceresti Health is a digital health company that has built a first-of-a-kind digital health platform, and conducted the clinical studies, to prove that activating family caregivers (i.e., improving the knowledge, skills and confidence of spouses and adult children) improves care and outcomes for complex and underserved Medicare members. The company’s non-medical platform leverages data and predictive analytics to activate and support caregivers via personalized education, proactive coaching, and remote monitoring, all delivered via a dedicated tablet that is shipped to the caregiver’s home.
