When we set up the first standardized international brain database to assess, train, and track patient outcomes, we had Collaborative Care Management (CoCM) in mind.
Now CoCM is a burgeoning reality. And I am delighted to be moderating a session on value-based outcomes in depression, with the founders of Kerna Health, a leader in digital connected care, on May 24 at this year’s Healthcare Innovation Congress.
We will discuss how despite the proven value of inter-connectedness; physical and mental healthcare delivery systems tend to operate independently. Individuals suffering from depression, anxiety, or substance use disorders for example are accustomed to disjointed healthcare. The consequences of this fragmented model can include gaps in treatment, inadequate care, poor patient outcomes and increased healthcare costs.
We will focus on evidence-based outcomes from integrating behavioral healthcare and primary healthcare within a primary care setting.
Together, we will discuss why a collaborative care approach to treating behavioral health concerns with comorbidities improves treatment outcomes and makes strong business sense; how providers can adapt collaborative care protocols across practice settings; build the necessary infrastructure; and secure insurance reimbursement. Further, we will explore recent case studies on how Kerna is integrating collaborative care across its patient and provider network
